	(ii)  Nature of injuries suffered in each cases and the details of medical aid given.	:
	(iii)  Duties on which these individuals were deployed at the time of accident.	:
5	Result of the inquiry made in the matter i.e due.	:
6	(a)  Action taken in reporting the matter to the local police /civil authorities and level at which the matter was taken up, indicating the time lag, if any between the time of occurrence and the reporting the matter to them.	:
	(b)  The FIR no registered with the local police authorities.	:
7	Action taken by the local police/ civil authorities against the defaulters.	:
8	Measure adopted locally with the aid of local authorities to prevent the occurrence such accidents.	:
